Subject to your informed consent, Qu Biologics Inc. (Qu) may request personal information, including name, sex, age, geographical location, contact number and current medical condition of individuals to determine eligibility for our clinical programs or for other purposes that would be disclosed prior to asking for your information.
For the Crohn’s Clinical Trial, Qu is requesting your personal information to make an initial determination about eligibility for participation in Qu’s upcoming Crohn’s Clinical Trial and to register individuals to receive updates regarding Qu’s clinical trials.

For the Crohn’s Clinical Trial, Qu uses your personal information to determine your potential eligibility to participate in the trial. Information submitted to Qu will be securely shared with the Crohn’s Clinical Trial site so that the site investigator can evaluate your potential eligibility for the Trial. The Crohn’s Clinical Trial site is restricted from using your personal information in any way other than providing clinical trial services for Qu.
Qu will take measures to protect your confidential information on secure servers, and will securely destroy or erase the personal information once it is no longer needed for the purpose for which it was collected.
DO WE DISCLOSE ANY INFORMATION TO OUTSIDE PARTIES?
We do not share, sell, trade, or otherwise transfer any personal information submitted to us except as disclosed to you when we request the information.
For the Crohn’s Clinical Trial, Qu will share your personal information only with the Crohn’s Clinical Trial site. If we believe that additional sharing of your information is warranted, we will ask for your consent to do so.
